Flight Instruction Safety: An AOPA Air Safety Foundation analysis
The AOPA Air Safety Foundation (ASF) studied more than 2,200 instructional accidents between 1992 and 2001. ASF found that most instructional accidents, whether dual or solo, occurred on takeoff or landing. Maneuvering flight, collision avoidance, fuel management, and continued VFR flight into IMC also earned coverage in the report, which includes tools and techniques for reducing the occurrence of these accident types.

Recommended flight instructor endorsements
To encourage standardization among flight instructors, the FAA provides recommended sample endorsements for the use of authorized instructors when endorsing logbooks for airmen applying for a knowledge or practical test, or when certifying accomplishment of requirements for pilot operating privileges. Here are instructions, and as well as the recommended endorsements, from AC 61-65E, Appendix 1.
